We believe it is essential to tour a facility to explore how well it meets your needs or those of your loved one. To learn how we can meet your individual needs and to arrange a personal tour, please call Admissions Coordinator Anne Goddard at (303) 789-2265, or please feel free visit our facility at your convenience and someone will be available to speak with you.
Soon after admission the interdisciplinary team will meet with you and your family to discuss your goals, and the plan of care developed by the team based on their overall assessments. This plan will be monitored and modified as necessary throughout your stay, whether long- or short-term. Family and caregivers’ involvement in this process is encouraged.
